Merchant Card Service - Apply Online
We recognize the need for getting your new account setup as fast as possible; our process takes less than 24 hours to have your account up and running.
To be assured that your merchant account will be setup correctly the first time; fill out our contact page and we will call you back within two hours to discuss the merchant service options you will need for your type of business, this will take approximately 10-15 minutes. If you are looking at a new or just changing processors and you are not currently taking Debit Cards, the saving are significant as to regular credit transactions, please view our reasoning here on why you should think about taking Debit Cards at your establishment.
To make it as easy as possible for you to complete the documents required to get a new merchant account. Click on the links (underlined words) to open each document. The documents are Adobe Acrobat PDF files allowing you to immediately type in your information in the appropriate fields. You will need to print these forms and then fax the required documents to the number provided on the fax cover sheet. You will also find a toll free number on the fax cover sheet, if you have any problems please call and we will walk you through the process. Ready to get started?
1. Adobe Reader 
Our documents have been created in Adobe Acrobat, except the Fax Cover Sheet, and you will need Adobe Reader in order to complete them on line. If you do not have Adobe Acrobat Reader please click on the Acrobat logo above and you will be directed to Adobe to download a free version of Adobe Acrobat Reader.
2. Merchant Application
It should only take approximately ten minutes to complete your application. Click on the Title above to open the application. You are able to type your information directly into the document fields. Print the completed application for your records; however, you only need to return the first two pages with the rest of your documents. To see an example of a completed application click SAMPLE .
3. Retail or MOTO/Internet Schedule A
You will need to print the appropriate Retail or MOTO (Internet) document and sign the bottom right corner.
4. Site Survey
You will need to complete the eight questions regarding your business location which we will review with you over the phone once received.
5. Chases Terms & Conditions
The 'Terms and Conditions' are part of the Merchant Agreement; however, we provide a link to them at all times for your convenience. It will be time stamped and available to you at any time if you misplace your printed copy.
6. Fax Cover Sheet
Our Toll Free fax number is (888) 300-7419. If you need assistance with any of these documents call us at (888) 361-8555. You need to return only the checked items: cover sheet, first (2) pages of the application, a voided check, Schedule A and the Site Survey Form. This should be a total of six pages.
Note: The Fax Cover Sheet when download will open as a Word Document, you will need to fill in the gray fields, by left clicking in the gray areas, and typing in your information. You can print this for faxing and then save this to your computer files.
